WELCOME TO WILDCAT ATHLETICS

BECOME ELIGIBLE:
In order to become eligible to participate in wildcat athletics, you must submit an athletic packet with the required forms and attend a mandatory parent meeting (1 per school year).

*THE ATHLETIC PARTICIPATION REQUIREMENTS MUST BE COMPLETED AND TURNED IN MINUMUM 1 BUSINESS DAY PRIOR TO COMING OUT FOR SPORT TO ALLOW TIME FOR PAPERWORK TO BE PROCESSED/CLEARED BY ATHLETIC TRAINERS
--> CONTACT SPORTS MUST ADDITIONALLY HAVE CURRENT IMPACT BASELINE ON RECORD

The Athletics paperwork required are:
1. Athlete physical exam form

Valid for 1 calendar year. (Ex: If your physical date was 9/1/2024, it expires ON 9/1/2025.)
Physical examination is done by physician
Please explain any "yes" answers in the medical history on the back page (Ex. "Have you ever had surgery?" Yes - tonsils removed, age 12yo)

2. Concussion management form

Valid for 1 school year.

(Ex: Valid for the 2025-26 school year.)

You MUST fill out the section "Voluntary" - you may choose to agree or not agree (voluntary participation), but filling out the entire form is required to be complete

3. Consent form

Valid per sport season.
1 consent form per sport - multiple sports cannot be listed on the same consent form

(Ex: If you are going to participate in football, basketball and track in the 2025-26 year, you will need to submit 3 consent forms)

4. Athlete, Parent Contract
