PERSONAL TRANSITION PLAN (PTP)
Frequently Asked Questions
Question 1: Is the Personal Transition Plan (PTP) a graduation
requirement?
YES. The Personal Transition Plan is a GRADUATION REQUIREMENT
for all students entering high school in the year (2006-07) and all incoming
students thereafter.
Question 2: What about the students who get good grades
and DON’T do the Personal Transition Plan? What will happen to them?
According to Hawaii State D.O.E. graduation requirements,
student who do not complete a Personal Transition Plan WILL NOT GRADUATE.
The PTP is worth ½ credit and is mandatory. Without completing
it, students will be half a credit short of graduating.
Question 3: What is a Personal Transition Plan?
It is a plan, presented in a portfolio (electronic or
hard copy), that will help your child make more conscious decisions in
relation to his/her future, whether it be college, the military, trade
school, or entering the work force. The PTP is not only designed to prepare
students for life after high school, but also to create more options for
students by encouraging them to explore different avenues for their future.
Question 4: Who is responsible when students don’t
meet the PTP requirements?
Ultimately, it is your child’s responsibility for
his/her own Personal Transition Plan. However, it is important for parent/legal
guardians, and our school to provide support to each student.
Question 5: Who determines whether the PTP components
were addressed?
The D.O.E, determines the required elements. Each school
determines the curriculum and implementation of the PTP. Schools may require
additional elements other than the minimum requirements. See http://graduation.k12.hi.us
Question 6: What topics/content will be addressed in the
Personal Transition Plan?
The elements of the PTP that have been determined are:
• Yearly Credit Checks
• High School Course Selections
• Interest, Skills, Work Values Assessments
• Career Portfolio
• Resume
• Prepare for Job Interview Process
• Post-Secondary Plan
• Personal Statement
• Formal Assessment (PSAT, SAT, ACT, ASVAB, AP Test, COMPASS, etc.)
• Best Works
Question 7: Will the PTP be hard copy or done electronically
on computers?
It can be done either as a hard copy portfolio or an electronic
ePortfolio. The majority of Konawaena student who are juniors and sophomores
already have an ePortfolio account and are working on their PTP. The current
freshmen will build their ePortfolio’s through their Career &
Life Planning class. Any student who does not have an account, regardless
of their grade level can receive an ePortfolio account at any time by
contacting the PTP Coordinator (see below).
Question 8: What about students who transfer into the
school. What will happen to them?
Since a PTP is required by all Hawaii public schools,
a student transferring into Konawaena from another Hawaii public school
should be in the process of developing a PTP. If the student is from an
outside school, they can complete the PTP from the point of enrollment.
However, Konawaena can determine if additional student work must be complete
to meet required benchmarks. If a student transfers out of Konawaena and
into another Hawaii public school, they will be able to transfer their
ePorfolio account to their new school.

Question 10: Are there any other new graduation requirements for the class
of 2010 and beyond? Where can I get information?
Yes. There are additional requirements, starting with
the class of 2010. Included are:
• High School Diploma: 24 credits (previously 22 credits were required)
• Board of Education Diploma: 3.0 GPA or above and completion of
a Senior Project.
